Klint insurance guide
What to do when a health claim is denied
Short answer: Start with the written denial reason, the plan language and the appeal instructions. An internal appeal or external review may be available, but the procedure and deadline must be verified for your plan and jurisdiction.
Find out exactly what was denied
Separate a coverage denial from a billing mistake, coding question, authorization issue or network dispute. Request the explanation of benefits and any denial letter; record the service, date, claim identifier and stated reason in your own secure records.
Ask the insurer or administrator which policy provision and appeal process apply. A customer-service conversation is useful, but written confirmation matters when you are tracking what was said.
Collect relevant records, not every private detail
- The denial notice and appeal instructions.
- Relevant benefit language and authorization information.
- Provider records that directly address the stated reason.
- A timeline of communications and submissions.
- The actual deadline from a verified notice or applicable official rule.
Internal appeal and external review are different
HealthCare.gov describes an internal appeal as asking the insurer to reconsider and an external review as review by an independent third party. Whether a pathway applies and the required sequence can depend on the plan and issue.
Marketplace eligibility appeals are a separate process from appealing an insurer’s coverage decision. Medicare, employer arrangements and other programs can have their own procedures. Do not assume one public summary covers every denial.
Get qualified help when timing or health is urgent
Ask the plan about any expedited process when appropriate. For immediate medical needs, contact the relevant health professional or emergency service; Klint is not a medical triage tool.
Klint’s checklist and correspondence draft are educational. Review them, verify dates, and decide what to submit yourself. Klint does not submit an appeal, guarantee reversal or provide legal representation.
